    • I understand that this is a general consultation intake form designed so that my stylist at The Cherry Blossom Salon can help me choose which new guest service options best suited for me.

    • I understand that I will need follow up by calling or texting the salon to schedule a virtual consultation time. During the consultation, my stylist will use this information to discuss a more thorough explanation of the coloring process and options.

    • I understand that by submitting this form it does not guarantee that my goal hair can be achieved in one session.

    • I understand that the pricing options and services will be discussed and will depend on my stylist, service choice, and that maintenance color appointments will depend on my current hair, goal hair, and required upkeep.

    By selecting the “I Accept” button, you are signing this Agreement electronically. You agree that your electronic signature is the legal equivalent of your manual signature on this Agreement. By selecting “I Accept” you consent to be legally bound by this Agreement’s terms and conditions.

    Salon Policies

    LATE CANCELLATION: If you need to reschedule/cancel, please give us a 24 hr notice so we may offer another guest the availability. Late cancels (less than 24 hrs) will require a 50% deposit for next booking.

    NO SHOW: When a guest does not come in for a scheduled appointment, it literally deprives the stylist of income for that day. Please call us to let us know if you need to cancel, to allow us an opportunity to offer another guest that time slot. If you no show, a 50% charge for the missed appointment will be added to your next service bill.

    CONFIRMATIONS: In support of our other guests, and your stylist’s time, please confirm your appointments when we send the confirmation text. A simple click or yes response will do it! Your appointment is subject to cancellation if we cannot confirm.

    SERVICE ADJUSTMENT: We strive to offer our guests the highest level of guest satisfaction. If you are having challenges with your cut or color, let us know within 10 days of your visit and we will be happy to correct the issue with no additional charge. We do not refund on services rendered.

    RETAIL RETURN: Should you make a retail purchase that you are unsatisfied with, we will accept retail exchanges at full credit for 90 days from your initial purchase.
